Voluntary life

      • Any employee who previously waived this coverage will require the EOI form to be completed.
      • Any enrolled employee wanting to add their spouse would be required to complete the EOI.
      • NO EOI is required to add children to the plan.
      • Any employee who is currently enrolled should be offered the GIB benefit without EOI (offer is greater of $10K or 10% to a max of 5x’s annual salary or $150K)
      • The employee must be actively at work to accept the GIB offer
      • Employees must be under the age of 70 to accept the GIB offer.
